Prepare for Your Visit

  1. Schedule an introductory phone call.

We offer free 10-minute introductory phone calls for anyone interested in our services. Take the opportunity to speak with one of naturopathic doctors, ask questions about our services, and learn more about naturopathic medicine. This call is complimentary and provides valuable insights into how we can help. It is entirely optional, and if you feel ready, you are welcome to schedule your first appointment directly.

  2. Schedule your first appointment.

In-person and virtual appointment types are available. After scheduling your first appointment, expect an email from Charm (our electronic health record system) to create an account and complete our new patient questionnaire before your appointment.

  3. Complete our new patient questionnaire online in Charm.

All new patients are required to fill out a questionnaire before their appointment. We find this approach enhances the care you receive as your doctor takes time to review your case before your appointment even starts. The notification to complete our questionnaire is sent to you through Charm to the email you provided. Once logged into the Charm patient portal, click ‘Questionnaires’ on the Home screen to fill out the form online.

[If you are unable to find the email from Charm, have difficulty logging in, or do not see a questionnaire in your portal, please contact us by calling 612-568-8382. As needed, paperwork may be completed at the time of your appointment.]

  4. Review our Notice of Privacy Practices.

We are required to maintain the privacy and security of your protected health information. You are asked to review and download our notice of privacy practices.

  5. Provide recent laboratory results, imaging reports, and other health records.

Within the Charm patient portal, you may upload your recent laboratory results, imaging reports, chart notes, or procedure reports for Dr. Panka to review in the 'Documents' section. Ensure that the documents you uploaded are shared with us within Charm. You can upload documents in formats such as .png, .jpeg, .pdf, and .doc under 50 MB. Please avoid uploading files in .docx or .html formats, as your provider is unable to view them.

You may also request a 'Release of Protected Health Information' from your other provider(s) and have them fax your medical records to us at (612) 568-9020.

  6. Payment is due at the time of service.

Please consider paying for services with cash, check, or bank transfer via emailed invoices to help us ease the impact of credit card fees on our small business. Your support in this matter is greatly appreciated! We do accept most major credit cards (Visa, MasterCard, American Express, Discover, JCB, and UnionPay), health savings accounts (HSA), and flexible spending accounts (FSA) as methods of payment.

Can I use my HSA/FSA card to pay?

Yes! We accept HSA and FSA cards as payment for services, labs, and supplements. However, there is no guarantee your HSA/FSA card is accepted for a particular charge by Panka Natural Health. If you run into any issues, the best thing to do is reach out to your card's bank or administrator to find out why. We can complete any required documentation to help ensure you receive coverage.

Are services covered by my health insurance?

Naturopathic doctors in Minnesota are out-of-network providers with health insurance. We are unable to contract with health insurance plans and bill them for our services. However, we can provide you with the information necessary for you to seek reimbursement from your insurer. Please let us know if you would like a superbill for your appointment. Medicare and Medicaid do not reimburse services provided by naturopathic doctors in Minnesota.

  7. Parking

Our address is 901 1st Street North Suite 901A Hopkins, MN 55343. We have ample off-street parking located in the parking lot in front of and behind our building. Our entrance is on the street side of Suite 901. We are conveniently located along the Lake Minnetonka LRT Regional Trail if you choose to walk or bike.

  8. Virtual Appointments

How to join the virtual appointment:

See the URL link to the virtual appointment in your appointment confirmation email. At the time of your appointment, click on the link following "Where." And then, "Ask to Join" the meeting. Dr. Panka will admit you into the virtual appointment when it's time to start. Please make sure your camera and sound are turned on.

There is a chance Dr. Panka will not be on-time for your appointment. Please allow a few minutes for your provider to join your virtual appointment as they may be finishing up another appointment. We try our best to block sufficient time between appointments to stay on schedule and prevent delays. We appreciate your patience.

If you have any issues joining the Google Meet, please call the office at 612-568-8382. If you have not joined 10 minutes after the start of your appointment time, Dr. Panka will give you a call for troubleshooting.

Note: If you would like to use your mobile device during a virtual appointment and do not have a Gmail account, please let us know in advance, and we can make accommodations through Google Duo. Look for the Google Duo App on your phone. If you do not see it, download Duo from the Google Play Store (Android) or AppStore (iPhone). Follow the on-screen set-up prompts, so you are prepared for your virtual appointment.

  9. Cancellation policy

We require 24-hour notice for any rescheduled or canceled appointment. No shows or canceled appointments within 24-hours of your appointment time will be charged a $50 fee.
